University Hospitals Kingston Foundation (UHKF) invites you to join us for free on Tuesday, October 27th at the Firehall Theatre in Gananoque for the latest addition of Talks with Docs.

Talks with Docs is a free educational series hosted by UHKF where community members can hear from and connect with leading health-care professionals across Kingston Health Sciences Centre (KHSC), Providence Care and the broader Kingston healthcare ecosystem! Each session focuses on a new topic, where speakers will provide their insights on the field and how care is advancing.

From Hospital to Home: Transforming Heart Failure Care in Southeast Ontario will feature the following speakers:

  • Dr. Aws Almufleh, Cardiologist and Physician Lead of the Heart Function Clinic, KHSC
  • Sam Gouett, NP Lead of the Heart Function Clinic, KHSC
  • Michael van Hartingsveldt, Superintendent of Community Paramedicine, Frontenac Paramedics

Together they will share how community-connected models of care are helping patients stay healthier at home, avoid hospitalizations, and live with greater confidence, and how donor support is helping make this possible.
